APEX Primary Care Network (PCN) is an organisation of four local GP Practices. Working together and covering a population of over 46,563 registered patients.
APEX PCN consists of a dedicated and hard-working team, who are striving to improve the health and wellbeing of our communities.
Our PCN consists of a variety of Clinicians from our Practices and other local services, which include Clinical Pharmacists, Frailty Practitioners, First Contact MSK Technicians and Social Prescribers and a Mental Health Practitioner. Together, our team have a clear insight to the needs and requirements of our community. With their expertise, we can target specific areas and take action to develop and improve services. We aim to support, encourage and educate our patients to be more proactive and take charge of their own health. To be healthier for longer, to develop better habits for cleaner lifestyles and with the potential of improving their quality of life.
Our Goal
Our goal is to assess the needs of our growing population, where the demand for local services are increasing and to educate our patients on the many local services that are available (in person and digitally).
